Advanced Directives

One of the most important steps you can take to assure that your choices are communicated if you can no longer speak for yourself is to prepare an Advanced Directive. This allows your family and healthcare providers to know what level of care you want, under the various circumstances that may occur toward the end of your life. We have copies of the “Five Wishes” and The Idaho Physician's Orders for Scope of Treatment. The State of Idaho recognizes both of these forms or you can have an attorney draw up a completely individualized form for you.
